The head of BPJS Kesehatan, Ali Ghufron Mukti, said that foreign nationals participating in the National Health Insurance (JKN) program contribute more in premiums than what is spent on their treatment.

“An interesting fact is that the contributions we receive from 124,000 foreign participants exceed the costs of their healthcare,” he noted. For example, in Bali, where about 15,000 foreigners are registered in the BPJS system, the monthly cost for their treatment is less than 1 billion Rupiah.
By law, participation in JKN is mandatory not only for Indonesians but also for foreigners officially working in the country for more than six months. This rule applies not to tourists, but to formal sector employees, from the hospitality industry to the mining sector. KITAS/KITAP holders who have lived in Indonesia for more than six months can also voluntarily subscribe to the insurance.
“We do not limit the number of foreign participants. Our goal is to provide medical protection for everyone who officially works in Indonesia, regardless of nationality,” emphasized Ghufron.
He added that foreign JKN participants live not only in Bali but also in Jakarta, Yogyakarta, Sulawesi, and other regions. Their premiums are calculated using the same scheme as for Indonesian citizens: 1% of the salary is paid by the employee, and another 4% by the employer.
Thus, the program remains financially sustainable while providing access to healthcare for both Indonesian citizens and foreigners legally working and residing in the country.
