An Australian citizen has been deported from Bali.

His old passport had expired, but he did not notify immigration about his new one or transfer his stay permit to it. He is now banned from entering Indonesia.
This transfer is called a passport mutation (mutasi paspor). Every foreigner who receives a new passport must complete one, whether they hold a KITAS, a KITAP or a visitor visa.
The application is usually submitted by the sponsor from their account on the immigration website while the foreigner is in Indonesia. It requires a letter from the sponsor and the sponsor's ID, pages from the old and new passports, the KITAS or KITAP, and the e-visa. A family KITAS requires more documents.
Without a passport mutation, you may not even be able to leave: there have been cases where foreigners were stopped at airport immigration control for this reason.
In total, Ngurah Rai Immigration deported seven foreigners last week. An Indian citizen tried to fly to Manila on a forged passport. A Russian passport holder with a remote worker KITAS was deported for prostitution, and four others for overstaying.

According to immigration officials, inspections look not only at what a foreigner is doing but also at whether their documents are valid.
You can have Legal Indonesia handle your passport mutation.
